COMMUNITY HEALTH CENTERS OF WESTERN KENTUCKY

A prebid conference will be held on September 5, 2023, at 10:00 AM, prevailing time, at the office of the owner, Community Health Centers of Western Kentucky, 480 Hopkinsville Street, Suite 2, Greenville, Kentucky 42345. All bidders should plan on attending this meeting.

Proposals will be received until 1:30 PM, prevailing time on September 19, 2023, at the office of the owner, Community Health Centers of Western Kentucky, 480 Hopkinsville Street, Suite 2, Greenville, Kentucky, and then at said office privately opened. Bidders will be notified of the results within one week.

Project Description: Construction of a single-story medical office building of approximately 8,000 square feet with a mechanical mezzanine.

The Work includes, but is not limited to site clearing, earthwork, termite control, hot mix asphalt paving, chain-link fences and gates, lawns and grasses, cast-in-place concrete, simulated stone masonry, unit masonry assemblies, structural steel, metal fabrications, rough carpentry, wood roof decking, metal plate connected wood trusses, glued-laminated construction, interior architectural woodwork, building insulation, asphalt shingles, siding, soffits, sheet metal flashing and trim, firestopping, joint sealants, standard steel doors and frames, flush wood doors, aluminum-framed entrances and storefronts, wood windows, door hardware, glazing, gypsum board assemblies, acoustical panel ceilings, resilient wall base and accessories, carpet tile, acoustic wall panels, painting, toilet compartments, signs, post and panel signs. fire protection specialties, protective covers, toilet and bath accessories, HVAC & plumbing systems, electrical (power & lighting) systems, and clean-up and removal of construction debris.

Proposals will be received as a Lump Sum Bid(s) for the work indicated in the Bidding Documents.

One set of the Bidding Documents may be obtained by contractors from Maco-Evansville Blue, 600 Court Street, Evansville, Indiana 47708 (812-464-8108), upon deposit of One Hundred Fifty Dollars for each set of Documents. Additional sets may be purchased at a cost of Two Hundred Dollars per set, non-refundable. Checks covering deposit shall be made payable to the Owner. All mailing charges (non-refundable), if required, are to be arranged in advance with and paid directly to Maco-Evansville Blue with a separate check.

Electronic copies of the documents are available from Maco-Evansville Blue, 600 Court Street, Evansville, IN. (812-464-8108). Charges to obtain electronic copies are non-refundable and are to be arranged with, and paid directly to, Maco-Evansville Blue.

Deposits will be refunded to contractors who submit a bona fide proposal to the Owner (directly or through a prime contractor) and return the documents, in an acceptable and reusable condition within ten days of the opening of the bids.
